In order to expedite our appointment process and ensure we continually run on time, we recommend all new patients download, fill out and bring in our new patient forms when they arrive for their scheduled visit. When coming for your visit, ensure you have a photo ID and your insurance card for validation.
We take most major insurance plans including, but certainly not limited to: VCP, VSP, Medicare, Medicaid, EyeMed, BCBS, Cigna, Davis, Spectera, MetLife, Avesis, Vision Care, VBA, Well Care, United Health Care, Universal, Cole, Healthfirst and CompBenefits.
For your convenience, we also accept the following for payment of patient balances or copays: Visa, Mastercard, American Express, Discover, debit cards, checks and cash.
It is our policy that full payment is due at the time services are rendered. We do offer lay-away plans on our frames, please inquire within our office for more details. We apologize, but we can order neither contacts nor eyeglasses without payment in full.
We submit claims to insurance companies with which we have a contract, however, it is the sole responsibility of the insured to know the type of insurance, coverages and any applicable copays or deductibles. Please understand that our office is not the cause of insurance delays or denials – if insurance is denied or reduced for any reason, the balance’s payment responsibility falls to you. Any balances that are over 120 days past due will be turned over to a collection agency and marked on your credit report unless previous arrangements have bene made. If you have a disagreement with your insurance company, you may submit your claim for reimbursement through them – remember, your insurance company is responsible to you, not us. Do not hesitate to contact them if you disagree with their payment or to find out the status of your claim.
Coupons, Specials, and Promotions
If you plan on using a coupon or internet-only promotion during your visit, please ensure you bring it with you (either bring in the magazine or print off the web page). We hope you can understand that we can only validate these promotions if you have the coupon to give us at the time of service. All coupons and promotions cannot be utilized with insurance benefits and they cannot be combined.
Non-emergency and routine appointments are usually made in advance, though we do accept walk-in patients if time is available. If you have a medical concern or emergency, call us as soon as possible and we will try our best to see you quickly. As a courtesy to all our patients, we strive to run on time. We reserve the right to reschedule or cancel patients who arrive 10 minutes or more later than their scheduled time. If you are running late, please notify our office as soon as possible.
Please give our offices 24 hours advanced notice if you need to cancel or reschedule, so we may open up your time slot to other patients needing emergency or walk-in care and attention.
Symptoms of emergency eye problems can include (but aren’t limited to): pain, foreign objects in the eye, flashes of light, loss of vision, “floaters” (little black specks floating across your vision), or injury (chemical splashes, blunt force trauma, etc). If you experience these symptoms, or any other that concern you, please contact our office as soon as possible. We will do our best to schedule you the same day. If you experience these when our offices are closed, at night or on the weekend, we have emergency care available 24 hours a day, seven days a week. Please call the office and follow the phone instructions. If you are calling about a true medical emergency, please call 911 immediately.
In consideration of our doctors, please only call our offices after hours if you have a medical concern — our on-call doctors cannot help you with prescription questions, glasses or contact lens replacement, glasses repair or contact lens ordering. Our offices are also unavailable for the pick up of your glasses or contact lens order after we are closed — please make arrangements to pick up your order during our regularly scheduled hours.
